Integrated delivery networks focusing on SNFs

Integrated delivery networks (IDN), faced with Accountable Care Organization (ACO) arrangements, bundled payments, and readmission rate concerns are looking at post-acute facilities, particularly skilled nursing facilities, to help with outcomes and costs.

For example, Managed Care reports that AtlantiCare, in southern New Jersey, recently queried 30 independent facilities to respond to a request for information, focusing on outcomes, process, structure, and existing perception.

AtlantiCare has specific questions and weighted each category differently to create an algorithm that provided six SNFs to be included in their system.

It’s worth noting that the recent waiver by CMS of the three-day acute-care hospital stay before patients can transfer to SNFs—but only if the SNF has received 3 stars or better on its STAR rating.
